Pregnancy Checklist:

• All pregnancies must be confirmed by sending a urine sample to the lab.
• NIMATS booking form to be completed at health centre once positive result is confirmed.
• An initial appointment is required with the Midwives usually between 8-10 weeks gestation. Each patient must contact midwives at Shantallow Health Centre on 02871321723 (9am-5pm) to request this appointment. If there is no reply, leave a message stating your name & address and that you need a 'booking appointment' - The Midwife will call back with appointment details.
